Privacy Policy

We reserve the right to protect all patient health information. As a private office we will only share medical information with the patient consent. This includes and is not limited to medical records, testing results, primary contact information, address, family members, treatment, diagnosis, email, or any form of information the patient wishes to keep private to our immediate office. Without permission from the patient via telephone or in person we will not and cannot share any information. Other health care organization asking for patient results or records must make a request via our fax number. We follow direction from our patient or a POA of patient only. Please help to keep patient health information secure and private. 

We follow and comply with all HIPPA guidelines and regulations. Simply put, if they do not want you to have it, you will not receive it from Agave Adult Medical Care.
