This unique plan is only available to 5 patients. This works off a first come first serve plan with possibly options of expanding after its first year. 

- Private connection with provider's direct cell phone for the year paid. 

        - Includes personal contact with provider for any and all needs. Provider to follow in the event of a emergency. Provider will be in tune with your every health care need at any time. 

        - The only time provider is not available is during major holidays and vacations. We will let the patient know ahead of time and check in to see if any services are needed.
